This Monthly Membership Agreement (Agreement) is made between Forrest Massage & Spa (the Spa) and the client (the Client) for the provision of massage and facial treatments. The Client agrees to the following terms:

1. Services: The Spa will provide the Client with a 60-minute massage or facial treatment each month. The Client may choose either a massage or facial treatment each month, subject to availability.

2. Lock-in Price: The Client shall enjoy a locked-in price of $75 for each 60-minute massage or facial for the duration of their membership. This price will not be affected by any price changes  that may occur during the membership period.

3. Membership Duration: The Client agrees to a minimum membership duration of one year (the Membership Period). The Membership Period will commence upon signing this Agreement.

4. Monthly Flat Rate: The Client agrees to be charged a flat rate of $75 per month for the duration of the Membership Period. This monthly charge will be automatically debited from the Client’s designated payment method on the same date each month.

5. Cancellation or Termination: If the Client wishes to cancel their membership before the completion of the Membership Period, a cancellation fee of $50 will apply. The Client must provide written notice of their intent to cancel at least 30 days prior to the desired cancellation date.

6. Benefits: The Client shall be entitled to the following additional benefits throughout the Membership Period:
a. 10% discount on any additional services or retail products purchased at the Spa.
b. Priority appointment scheduling, subject to availability.
c. Ability to transfer the unused monthly treatment to a friend or family member upon prior notice to the Spa.

7. Availability: The Spa will make its best efforts to accommodate the Client’s preferred appointment times and therapists. However, appointment availability is subject to demand and may vary.

8. Non-Transferable: The Membership is non-transferable and may only be used by the named Client.

9. Changes to Terms: The Spa reserves the right to modify the terms of this Agreement with written notice to the Client. The Client shall have the option to cancel their membership within 30 days of receiving such notice if they do not agree to the revised terms.

10. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state of Florida.

By signing below, the Client acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Monthly Membership Agreement.
