This Annual Membership Agreement (Agreement) is made between Forrest Massage & Spa (the Spa) and the client (the Client) for the provision of massage and facial treatments. The Client agrees to the following terms:

1. Services: The Spa will provide the Client with discounted massage and facial treatments each year. The Client may choose from the following options, subject to availability:
a. 60-minute massage or facial for $85 (28% off regular price).
b. 90-minute massage or facial for $119 (25% off regular price).

2. Pay One Time Yearly Fee: The Client shall pay a one-time yearly fee of $50 for the duration of their membership. This fee covers administrative costs associated with the membership.

3. Annual Flat Rate: The Client agrees to be charged a flat rate of $50 per year for the duration of the Membership Period. This annual charge will be automatically debited from the Client’s designated payment method on the same date each year.

4. Cancellation or Termination: If the Client wishes to cancel their membership before the completion of the Membership Period, a cancellation fee of $50 will apply. The Client must provide written notice of their intent to cancel at least 30 days prior to the desired cancellation date.

5. Benefits: The Client shall be entitled to the following additional benefits throughout the Membership Period:
a. 10% discount on any additional services or retail products purchased at the Spa.
b. Priority appointment scheduling, subject to availability.
c. Ability to transfer the unused yearly treatment to a friend or family member upon prior notice to the Spa.

6. Availability: The Spa will make its best efforts to accommodate the Client’s preferred appointment times and therapists. However, appointment availability is subject to demand and may vary.

7. Changes to Terms: The Spa reserves the right to modify the terms of this Agreement with written notice to the Client. The Client shall have the option to cancel their membership within 30 days of receiving such notice if they do not agree to the revised terms.

8. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the State of Florida.

By signing below, the Client acknowledges that they have read, understood, and agreed to the terms and conditions outlined in this Annual Membership Agreement.
